Anticipatory Care Planning (ACP)

What is Anticipatory Care Planning? (ACP) 

An opportunity for your patients/clients to plan ahead for any change in their health or social care needs. It aims to put the needs and preferences of the people, who use our services at the centre of decisions about their care, and to communicate and record this information effectively.

What are the benefits of ACP? 

ACP promotes person-centred care (main principle of this is that it depends on the needs, circumstances and preferences of the person receiving care). It:

  • improves the possibility for person choice
  • empowers people to make difficult decisions
  • improves people's experience
  • supports people to live in their preferred place of care
  • prepares the patient and family to manage on going health.

What might an ACP discussion involve? 

  • Any concerns the person may have about their health and social care needs
  • Anything the person thinks is important to them

Key principles in ACP 

  • It is voluntary
  • It can be offered to every patient who may benefit.
  • The ACP process puts the person at the centre of the plan and can be discussed over time.
  • If the person does not wish to discuss some questions, this should be respected.
