Refresher Training Notification Form

As a Personal Licence Holder you need to undergo refresher training every 5 years for the date your licence was issued. You must provide a copy of your training certificate to the Licensing Board no later than three months after each 5 year training period.

If you have recently completed your refresher training you can use this form to send the Board a copy of your training certificate (SCPLH-R).

To use this form you will need an electronic copy of your training certificate. This could be a PDF or scanned copy of the certificate or alternatively a clear photo of the full certificate is acceptable. If you are using a device with a camera, you will be able to take a picture of the certificate and attach it.

If you do not have an electronic copy of your certificate you can download our Notification Form and post the form and certificate to us.

In order to complete this form you will need to supply us with personal data.
